
En la última década, hemos asistido a una revolución en el tratamiento del cáncer de próstata resistente a la castración metastásico (CPRCm), una forma avanzada y letal de cáncer de próstata. Gracias al mayor conocimiento de sus características biológicas subyacentes, se ha podido desarrollar una serie de compuestos dirigidos a la vía de señalización andrógena y el sistema inmunitario, así como taxanos y radiofármacos.
Pese a los avances encaminados a manejar el CPRCm de manera eficaz, esta continúa siendo una enfermedad letal con una tasa de morbilidad y mortalidad importante a escala mundial. Se puede decir que la necesidad más imperiosa en el desarrollo de fármacos contra el CPRC es la estratificación molecular del tratamiento. De forma paralela, los esfuerzos deben seguir orientándose a identificar biomarcadores predictivos de la respuesta sólidos y al desarrollo de tratamientos antineoplásicos dirigidos. La llegada de estos novedosos tratamientos ha impulsado la evolución de los tumores hacia un cambio en el panorama genómico observado en los pacientes con un estadio avanzado de la enfermedad.
Adoptamos un enfoque exhaustivo e integral en la investigación. Como tal, nuestro grupo engloba la biología molecular, la genómica del tumor, los ensayos clínicos y la ciencia computacional a fin de desarrollar estrategias de medicina de precisión para el tratamiento del cáncer de próstata avanzado a partir de biomarcadores predictivos de la respuesta.
Entre el 20 y el 25 % de los casos de CPRCm presentan anomalías en los genes reparadores de daños del ADN (DDR, por sus siglas en inglés), especialmente en las roturas de doble cadena, y nos permiten estudiar el modo de ofrecer tratamientos y cuidados más precisos para el tratamiento del cáncer. Algunas de estas mutaciones conllevan unas implicaciones pronósticas y predictivas decisivas para hacer realidad la promesa de una medicina personalizada en oncología. Hemos publicado los resultados finales del ensayo para el registro de olaparib (un inhibidor de PARP) en el cáncer de próstata, lo que ha dado lugar a su autorización en Europa, Canadá y los Estados Unidos, entre otros países. Aun así, es necesario seguir trabajando para conseguir una oferta adecuada de tratamientos con inhibidores de PARP para los pacientes con mutaciones del gen DDR aparte de BRCA2.
El interés por la modulación del ciclo celular a partir de los daños del ADN también nos ha llevado a estudiar el fenotipo senescente observado tras la exposición a fármacos dirigidos, que suponemos un mecanismo de resistencia farmacológica, y el modo en que podemos actuar sobre él por medios terapéuticos. Con el propósito de transformar nuestros hallazgos en beneficios para los pacientes del modo más rápido posible, estudiamos las mismas firmas genómicas y transcriptómicas en biopsias de pacientes con cáncer de próstata metastásico. Simultáneamente, tomamos biopsias líquidas longitudinales para estudiar cómo evoluciona el tumor durante la respuesta y la progresión a los fármacos selectivos. Nuestra investigación se centra en las estrategias adecuadas de estratificación de pacientes para la atención clínica, con especial hincapié en la combinación de fármacos dirigidos a la reparación del ADN con los inhibidores de la señalización de andrógenos. Cabe destacar que hemos puesto en marcha un primer ensayo clínico iniciado por el investigador para traducir los estudios en el laboratorio en intervenciones terapéuticas para los pacientes.
- Establecer una reclasificación de importancia clínica del cáncer de próstata metastásico que integre datos genotípicos y fenotípicos con ensayos funcionales.
- Llevar a cabo ensayos de estratificación molecular del cáncer de próstata a partir del tejido tumoral y los biomarcadores circulantes.
- Optimizar la combinación de fármacos dirigidos a la reparación del ADN con inhibidores del receptor de andrógenos.
- Crear un núcleo de medicina de precisión para los pacientes con cáncer de próstata tratados en el Hospital Universitario Vall d’Hebron (HUVH).
