Las inmunoterapias contra el cáncer aprovechan el sistema inmunitario para atacar la enfermedad. Los estudios clínicos han demostrado que los inhibidores de los puntos de control inmunitarios y los tratamientos a partir de linfocitos T pueden intervenir en la regresión del tumor en pacientes con enfermedad metastásica. Así, pues, además de la cirugía, la radioterapia y la quimioterapia, la inmunoterapia representa cada vez más el cuarto pilar del tratamiento oncológico. Nuestro grupo se centra en conocer mejor la respuesta natural de los linfocitos T al cáncer y determinar maneras de aprovechar estas respuestas antitumorales para desarrollar inmunoterapias contra el cáncer más eficaces, potentes y personalizadas.

La creciente evidencia disponible indica que los neoantígenos desempeñan un papel central en la eficacia clínica de las inmunoterapias contra el cáncer. Con el apoyo del Programa Integral de Inmunoterapia e Inmunología del Cáncer de la Fundación BBVA (CAIMI-I y CAIMI-II), así como de otras agencias financiadoras, y gracias a nuestra larga colaboración con Elena Garralda, jefa del Grupo de Desarrollo Clínico Precoz de Fármacos del VHIO y directora de la Unidad de Investigación de Terapia Molecular del Cáncer (UITM) – CaixaResearch, estamos llevando a cabo un ensayo clínico de fase I para evaluar la seguridad y tolerabilidad de los linfocitos infiltrantes de tumor (TIL) seleccionados por su reactividad frente a neoantígenos.

Este ensayo clínico en curso, financiado por el Instituto de Salud Carlos III (ISCIII), emplea una estrategia altamente personalizada (figura) para detectar el reconocimiento de antígenos mutados mediado por células T. En este estudio piloto, nuestro objetivo es tratar a un máximo de 10 pacientes con cánceres epiteliales y melanoma refractarios a los tratamientos estándar. Mediante el enriquecimiento de linfocitos reactivos frente a neoantígenos, buscamos ampliar la eficacia clínica de la terapia con TIL más allá del melanoma.

Paralelamente, nuestro grupo participa en otros ensayos clínicos en colaboración con Elena Garralda y diferentes centros académicos de Barcelona, en los que se evalúan TIL no seleccionados y expandidos ex vivo para el tratamiento del melanoma, los tumores de células germinales y los cánceres con mutaciones en el complejo SWI/SNF. Entre ellos, el ensayo PragmaTIL es un estudio multicéntrico financiado por el programa Horizonte Europa cuyo objetivo es reducir la toxicidad asociada a la administración de dosis elevadas de IL-2. Cabe destacar que este ensayo supone una importante innovación regulatoria, dado el carácter descentralizado de la producción de TIL.

Uno de los objetivos estratégicos de nuestro grupo es desarrollar estrategias que permitan aprovechar el repertorio circulante de linfocitos reactivos frente al tumor. Hemos demostrado que las células T reactivas frente al tumor pueden detectarse con frecuencia en la sangre periférica de pacientes con cáncer, independientemente del tipo de tumor. La posibilidad de rastrear y monitorizar en sangre las células T CD8⁺ y CD4⁺ reactivas frente al tumor presenta un considerable potencial terapéutico, aunque este enfoque sigue siendo complejo debido a la baja frecuencia de estas células. Hemos contribuido a definir una firma fenotípica que permite identificar y enriquecer células T reactivas frente a tumores y neoantígenos a partir de sangre periférica.

Asimismo, como parte de la transición hacia estrategias mínimamente invasivas, hemos demostrado que el ADN tumoral circulante (ctDNA) puede utilizarse para identificar neoantígenos, lo que proporciona un método no invasivo para caracterizar antígenos específicos del tumor. Además, estamos desarrollando un flujo de análisis computacional para la caracterización integral, a nivel de célula única, de las respuestas de las células T de sangre periférica en pacientes tratados con inhibidores de puntos de control inmunitario (ICI) y vacunas contra el cáncer, con el objetivo de comprender mejor los factores determinantes de la respuesta al tratamiento.

Nuestro grupo también está comprometido con el desarrollo de tecnologías de alto rendimiento que permitan descifrar el panorama de los antígenos tumorales que aumentan la susceptibilidad de las células cancerosas al ataque inmunitario, así como avanzar en el conocimiento de los neoantígenos compartidos y de los mecanismos de resistencia a la citotoxicidad mediada por células T.
