Grup d'Expressió Gènica i Càncer

Estudiem tumors cerebrals primaris i metàstasis cerebrals, que són alguns dels càncers més agressius. Tant el glioblastoma com la metàstasi cerebral són malalties molt greus amb opcions terapèutiques limitades. Per això, és fonamental avançar en aquest camp per millorar els resultats d’aquests pacients.

El nostre grup està dissenyant eines per supervisar i descriure tumors cerebrals. Hem descobert que podem descriure els tumors cerebrals mitjançant l’anàlisi de les cèl·lules immunitàries i l’ADN tumoral circulant lliure de cèl·lules al líquid cefaloraquidi. Tot i que encara no s’ha integrat a la pràctica clínica cap biomarcador derivat de la biòpsia líquida d’aquests tumors, les proves cada cop més nombroses publicades, incloses les nostres troballes, apunten a la seva eficàcia per avaluar en temps real la malaltia maligna i al seu potencial per informar i guiar millor el tractament terapèutic dels pacients.

Com a reflex del nostre treball en aquest camp, se’ns va convidar a participar en la redacció de dos articles de revisió (que incloïen recomanacions de pràctica clínica) sobre les aplicacions clíniques de les biòpsies líquides al càncer cerebral (Soffietti et al. 2022; Pascual et al 2022) i escriure un article News & Views a Nature Reviews Clinical Oncology (Seoane i Escudero 2022).

Estem compromesos amb l’avenç de la investigació per desenvolupar nous compostos terapèutics per tractar el càncer cerebral. El 2022, informem del desenvolupament preclínic de dos nous compostos que ara estan en fase de desenvolupament clínic:

1- Amb l’ús de models derivats de pacients, vam demostrar l’eficàcia i el mecanisme d’acció d’un nou inhibidor de BRAF permeable al cervell en el context de melanomes amb mutació de BRAF que van recaure amb el tractament de referència (Bonfill-Teixidor et al .2022). Actualment, un assaig clínic per provar el compost està reclutant pacients al nostre Hospital (ISRCTN13713551).

2- També amb l’ús de models derivats de pacients, estudiem els mecanismes d’acció d’un nou anticòs terapèutic biespecífic (EGFRvIII TCB), que s’uneix tant als limfòcits T CD3 com a la mutació somàtica EGFRvIII expressada en el 20%-30 % dels glioblastomes. S’han publicat els resultats (Iurlaro et al. 2022) i el resum gràfic (vegeu la figura) apareix a la portada de la revista. Aquest estudi va ser la base d’un assaig clínic que s’està duent a terme al VHIO (NCT05187624).

El paper del microambient tumoral que, en el cas del càncer cerebral, assumeix un paper crucial a la progressió del càncer. Els avenços en el coneixement del microambient tumoral prometen armes poderoses per combatre el càncer, independentment de la seva heterogeneïtat. En aquest context, hem escrit un article sobre els avenços i les limitacions d’una nova substància anticancerígena dirigida contra TGF-β i PD-L1 (Gulley et al. 2022).

També hem informat que la citocina LIF assumeix un paper essencial al microambient tumoral i, en conseqüència, constitueix una prometedora diana terapèutica. Hem descrit el mecanisme d’acció del nostre nou fàrmac MSC-1 (ara AZD0171) (Hallett et al. 2022). MSC-1 és un anticòs neutralitzant de LIF desenvolupat per l’empresa derivada del VHIO, Mosaic Biomedicals, fundada per Joan Seoane el 2012. Medimmune/Astrazeneca va adquirir Mosaic Biomedicals el 2020. El 2022, vam publicar els resultats de l’assaig clínic de fase I et al 2022, NCT03490669). El 2021, es va iniciar el reclutament de pacients per a un assaig clínic de fase II (NCT04999969) per provar el MSC-1.
